    I have noticed more and more 377s and 387s with faulty sockets recently. I think often these things just never get reported. If it is just one socket broken (rather than an issue with the whole carriage) then it probably never gets noticed by the depot and then does not get fixed.

    They are already dual voltage. The entire fleet of 387 units were built as dual voltage units with both third rail shoes and pantographs. No modifications are required.

    That is often a possibility for these types of crashes. But it was reported in the BBC that the driver had mentioned to someone at the scene that the accelerator was stuck and he could not work the brakes. So it does not sound like a medical incident to me.
