For any G27 owners out there, I figured this out the other day as I was bored of playing TS2015 with the keyboard.

Here's a video of it in action. It's obviously not as realistic as a RailDriver controller, but feels quite authentic for trains with a combined handle.

You can move the power handle notch-by-notch by clicking the shifter forwards or backwards then returning it to centre. If you leave the shifter in a gear position, the lever will continue to notch up/down in that direction.

Essentially it works by interpreting presses of the gear buttons as keystrokes. This is achieved using the ControlMK app and assigning keystrokes to the buttons. For anyone who wants to do the same, my ControlMK assignments are:

Button 10 (3rd gear): Key D, repeat set to 15/second.
Notch forwards (reduce throttle/increase brakes)
Button 11 (4th gear): Key A, repeat set to 15 per second.
Notch backwards (increase throttle/release brakes)
Button 0 (1st red button): Key Q, repeat set to none.
AWS/Vigilance
Button 3 (4th red button): Key T, repeat set to none.
Doors
Button 15 (Topmost black button, above d-pad): Key Space, repeat set to full.
Horn
Button 17 (Bottom black button, above d-pad): Key Esc, repeat set to none.
Menu

I hope this is of some use to someone - it doesn't take long to set up at all. :) The hard part was figuring out how to make it happen!
