Looks like the cover from New Order's 1983 12" single "Blue Monday".

Exactly. Styled to look like a 5.25” floppy disk, complete with die-cut holes that made the sleeve so expensive to produce that Factory Records lost money on every copy they sold. (Un)fortunately for them, it became the biggest selling 12” single in history.

The sleeve doesn’t have the name of the band or song printed on it, but the coloured shapes on the right hand side spell it out in a code which is revealed on the back of the Power, Corruption And Lies album

The recording studio is all yours....
